Yes, Anagen stimulators are ABSOLUTELY important!

Below are grafts from a donor area taken recently by our CIT version of FIT. The patient relayed a story that a month after beginning Propecia he noted shedding in his donor area. He stated that each morning when he awoke, he began seeing 12 or more hairs on his pillow. He also stated that he began seeing more hair come out when he ran his hands through the back of his scalp. He clearly stated that the shed hairs were from his donor area.

Examination of the grafts revealed a 17% telogen ratio. This is high for a donor area, but not beyond the normal limits. According to Kligman, one can have up to 25% telogen hairs on the scalp.

The history was very suggestive of an increased telogen ratio and this is what we found. Typically there is no more than 8 to 12% telogen hairs. Propecia stimulates anagen. When a new anagen cycle begins, the old resting hair is pushed out of the scalp and onto the pillow. The shift to anagen suddenly by propecia was evidenced by increased shedding.

Incidentally, each hair has only so many hair cycles. This is why you want to load up on anagen stimulators designed to prolong or promote anagen. These include Propecia, Avodart, Minoxidil, Nizoral, and Hair Cycle Shampoo. Multiple products used concurrently probably work better than one product at a time.

I'm confused. What is indicative of a higher telogen ratio? Obviously it sounds like less hair is in the growing stage, but does that have anything to do with hair loss?

Is it correct to say that anagen stimulators like propecia will lower this patient's telogen ratio over time? For instance, the patient is at around 17% does that mean it should come down to the more "normal" range of 10-12%?

Also I'm confused about the significance of shedding in the donor area. Thanks,

To find the answer, you must measure the density of a donor area. Once you have the this measurement then asses the overall number of the telogens (much thinner hairs). You will have a higher telogen ratio if the average hair follicle diameter of a specific area within the donor region is small.

In hair loss, anagen stimulators enable you to keep your hair for a longer term. No one knows exactly how many cycles his or her follicles will have. Over time, I would say that the patient's telogen ratio will become lower after consistently using anagen stimulators because the frequency of hair cycles into the telogen phase should be lower if most of the hair is still in this "prolonged anagen phase". Most areas that are effected by DHT would be positively effected. Loss of hair in the donor is an indication of new hair sprouting through the dermis. Would a younger guy who begins using these products (hair cycle) after he has a transplant benefit the same as a younger guy using these products before having a hair transplant?

I say that guys would benefit the same. post op HT individuals could keep terminal hair longer then virgin scalps could hold on longer to their NW class.
